Physio preferred providers terms
To receive the physio no-gap kids offer, the patient must be a Union Health member, aged 17 years and under, with Extras cover that includes physiotherapy with an available benefit limit and be eligible to receive a benefit. Participation in the $90 initial consultation offer is deemed acceptance of these terms and conditions and can only be offered once per year per Union Health member. The item number subject to no gap payment is 500 for an initial consultation. To receive the physio 10% off offer, the patient must be a Union Health member, with Extras cover that includes physiotherapy with an available benefit limit and be eligible to receive a benefit. Participation in the 10% off consultations offer is deemed acceptance of these terms and conditions. If services beyond an initial and or standard consultation are provided, your practitioner will advise of you of the additional cost before offering these services. Excludes services for massage, paediatric physio, Pilates, hand therapy, occupational and speech therapy, hydrotherapy and other allied health services not included as physiotherapy. Offer cannot be used with any other offer, discount, and voucher or government scheme. Offer is subject to any health fund rules and is not redeemable for cash. Valid for new and existing North West Physiotherapy patients from 1 December 2022. Please note that if you forget to bring your health fund membership card to your appointment, you will need to pay your account in full and then claim your benefit directly from Union Health.
Podiatry preferred providers terms
To receive the podiatry no-gap kids offer the patient must be a Union Health member, aged 17 years and under, with Extras cover that includes podiatry with an available benefit limit and be eligible to receive a benefit. Participation in the $90 Comprehensive consultation offer is deemed acceptance of these terms and conditions. Item numbers subject to no gap payment are F004, F118, F301 and F301 for scan. To receive the podiatry 10% off offer, the patient must be a union Health Member with Extras covers that includes podiatry with an available benefit limit and be eligible to receive a benefit. Participation in the 10% off initial consultation offer (excluding kids) is deemed acceptance of these terms and conditions. The item numbers subject to the 10% off offer is F004. If services beyond an initial consultation, muscle & range of movement testing, gait analysis are required, your podiatrist will advise of you of the additional cost before offering these services. Offer cannot be used with any other offer, discount, and voucher or government scheme. Offer is subject to any health fund rules and is not redeemable for cash. Valid for new and existing North West Physiotherapy patients effective from 1 December 2022. Please note that if you forget to bring your health fund membership card to your appointment, you will need to pay your account in full and then claim your benefit directly from Union Health.
